NEUROLOGICAL MEDICAL OUTPATIENT REHABILITATION PROGRAM
THE NEUROLOGICAL PROGRAM:
This is a unique neurological outpatient rehabilitation program which offers a multi-discipline approach to treat adults who have suffered from neurological and medical diagnoses. The program is designed to evaluate and treat physical, cognitive, communication, emotional and behavioral problems associated with medical and neurological diagnoses. The focus is on improving an individual’s ability to perform daily self-care, homemaking, leisure and work within their communities. Treatment is provided individually, in groups or on outings in the community where individuals are able to practice and adapt their new skills in real-life situations. Successful community integration is our goal!
Imbedded in this Neuro program is the ESTEEM program, which is CARF accredited for treatment of people with traumatic brain injuries. ESTEEM is an acronym for Establishing Success after Trauma, Evaluation, Education and Management
OUR STAFF:
Our team of dedicated staff is sensitive to the physical, emotional and behavioral changes that occur following an illness or neurological diagnosis. Each person’s program will be tailored to address and respect the person’s individual needs and goals, cultural and spiritual beliefs.
Our team includes Physical, Speech and Occupational therapists, Neuropsychologists and when appropriate, a Vocational specialist. We have a Nurse and Case Manager to facilitate medical education and community liaison for resources for you and your family.
WHO IS ELIGIBLE FOR OUR PROGRAM?
- Individuals who have a medical or neurological diagnosis, including, but not limited to:
- Spinal Cord injuries
- Guillian Barre’
- Peripheral Neuropathies
- ALS
- Multiple Sclerosis
- Low vision
- Balance Disorders
- Incontinence
- Parkinson’s Syndrome
- Laryngectomy
- Pre-driving Assessments
- Voice disorders
- Stroke
- Traumatic Brain Injury
- Other diagnoses upon review
- Individuals must be at a level of function allowing them to travel to an Outpatient setting and who have access to private or community transportation to and from the clinic.
DISCHARGE CRITERIA:
- An individual will be discharged from the program when the therapist feels they have met their goals and/or achieved maximum benefit from the program. Home exercise programs, referrals to community support groups and agencies will be set up to facilitate ongoing integration in the community.
- An individual may be discharged at the end of their authorized treatment period, as per their funding source. Further financial options may be explored as needed.
- Individuals are expected to attend all scheduled treatment sessions. In the event there is a scheduling conflict, we ask that the individuals call to cancel or reschedule. We will discharge individuals who consistently cancel or miss 3 consecutive appointments without communicating with us.
- Our programs are drug-free environments. Evident abuse of drugs and alcohol while engaged in this program will result in discharge.
PROGRAM QUALITY ASSURANCE:
The Medical Outpatient program reviews program quality and outcomes on a quarterly and annual basis. We compare the outcomes of our discharged patients against the outcomes of similar patients in our system. We are able to use this information and comparisons against previously discharged patients to predict outcomes for our current patients. We use our outcomes to constantly strive to improve our programs and improve our patient satisfaction. This outcome information is available upon request.
Our patients are surveyed after discharge by telephone at 30 and 90 days post discharge to discuss the carryover of their functional gains. Our patients are well prepared for maintaining their functional levels after discharge. - Medicare and Medicaid
- Commercial insurance
- Workers Compensation/Vocational Rehabilitation
- Florida Brain and Spinal Cord Trust Fund
- Cash accounts as pre-arranged with Winter Haven Hospital
