Orthopedic/Medical Outpatient Program

Orthopedic/Medical Outpatient ProgramThe services provided for the patient with medical or orthopedic- musculoskeletal diagnoses include Occupational and Physical therapies. We treat many diagnoses including back, shoulder and knee injuries, amputations, lymphedema of the arm, balance disorders to name a few. We employ certified hand therapists who work closely with area hand surgeons on difficult hand, elbow and shoulder injuries.

Our program employs an RN/Case Manager and a Vocational Evaluator to assist with medication education and connecting with community resources for the patient and family.

Orthopedic/Medical Outpatient ProgramOur facility is an outpatient setting that is accessible to the public and is open Monday through Friday from 7 AM to 7 PM. Our patients are referred by physicians for outpatient rehabilitation and are physically able to access an outpatient center. The majority of our patients travel to the Center by car or public transport. The Case Manager may assist in finding transportation for those who are eligible for public assistance.

Winter Haven Hospital accepts Commercial insurance companies, Workers Compensation, Medicaid and Medicare as payers. We arrange for cash accounts as required. We are a not-for profit organization. Our Case Manager or Patient Services Coordinator may assist in gaining funding for treatment and equipment needs.

Our patient population is 13 years old through adult with physical impairments affecting performance of activities of daily living, mobility, sports activity and work. Our adolescents are seen for orthopedic sports related injuries including ACL repairs, fractures or strains and sprains.

Orthopedic/Medical Outpatient ProgramOur team will provide assistance in returning patients to their prior level of community activity. The team is sensitive to cultural and spiritual issues that may arise with a change in a person’s abilities. Referral to a Psychologist or the Behavioral Health Division is possible for people experiencing difficulty with their physical rehabilitation. If chemical or drug abuse issues become apparent during rehab, referral to the Behavioral Health Division may be made.

The Medical Outpatient program reviews outcomes of their past populations and we predict outcomes for the individual based on their entry level of impairment and the outcomes of similarly diagnosed persons seen in this program. We look at national data for similar diagnoses and use this data to measure our success.

Outpatient Rehabilitation Center

Admission Criteria

  • Persons served age 13 and older
  • Diagnosis as appropriate for outpatient services for musculoskeletal/orthopedic rehab
  • Medically stable and able to actively contribute in an Outpatient Rehab Program
  • Ability to benefit from services
  • Identify funding source, or receive approval from Administration/Finance Department.
  • Not harmful to self or others
  • Be drug and alcohol free for duration of the program

Discharge Criteria

Discharge criteria are established with each person served on initial evaluation.

Discharge criteria for OPRC.

  • Person served has reached their maximum benefit from therapy or have achieved their long-term goals.
  • Person served is non-compliant with their program through lack of attendance (3 cancellations or 2 no-shows). Person is referred back to their physician with recommendations.
  • Person is independent in a home program and no longer requires skilled care.
  • If person is at a level of functioning, which is better served in another program or agency, they are transferred and discharged from this program
